• The applicant is subject to the Virginia Procurement Act.
• While price will be the primary consideration, points will be awarded to vendors holding state master, cooperative, or GSA contracts. If utilizing a cooperative contract, a copy of the contract or a link to a digital copy should be included with the response.
• Any applicable installation, activation, or initial configuration fees must be included with the response.
• All proposed pricing should be embedded within any bid proposal.
• Proposals for multi-year contracts and contracts with optional renewals will be considered, but are not required.
• Subject to contract restrictions, services may be reevaluated for cost-effectiveness at any time during the year. Bids for services requested, therefore, will be accepted up to 28 days after the posting of next year's Form 470.
• As required by E-rate rules, all bids in response to this Form 470 must offer the Lowest Corresponding Price (LCP). See http://www.usac.org/sl/service-providers/step02/lowest-corresponding-price.aspx.
• To assure full consideration, bids and/or information requests directed to the applicant contact provided in this Form 470 should (a) reference the nine-digit Form 470 number and (b) be copied to Melissa Zaruba at E-Rate Central by fax (516-801-7854).

There are two separate internet connections that need to have firewall and caching services. Prefer a subscription based service that have both caching and firewall services and unified user interface. Quotes for district purchased appliances should include three year warranty, service and update services as allowed by E-rate rules. Additional non- E-rate allowed services such as web filtering may be included, but must be clearly identified and broken out on the quote. Systems should be plug and play or configuration to work with the current network included in the service. |

Mathews County Public Schools is looking for 10 - 20 48 port POE+ switches in a mix of 1gb and 10gb capable copper ports. 2- 5 switches should have two fiber uplink modules with gbic’s for each (4-10 total). At least two of the gbics should be multimode and 2 should be single mode. For the 2-5 switches we prefer all the copper ports be capable of 10gb but will consider a mix of 1gb and 10gb capable ports. We will also consider some 48 and 24 port models that are stackable, have a high speed backplane and are non-POE for the other switches. 2-5 switches should have advance routing licenses included in the price quote and the OS of the switches should be the similar if possible. All switches should include three year warranty, updates and service that meet E-rate allowable components that can be included with category 2 equipment purchase. This would include any modules installed at time of delivery. We prefer not to use add-on modules/blades for additional ports and functionality and would like everything built into the switch model chosen for the quote. (with the exception of the gbics) We would also like any feature that would allow the stack group routing and functionality to continue working with the loss of a member switch. If additional information is needed please request this via email with this 470# noted in the subject line. |
